Alive & Well at Home wants to assist our community with aging and recovering in place with dignity and grace. Our clientele seeks exceptional service and care. We specialize in skilled nursing services, proactive wellness and non-medical related health services.
We offer skilled nursing care, plan of care management, and assistance with activities of daily living.
Some of our services include transporting to/from medical visits. Attending healthcare provider visits and supplying visit summaries. Assessment of available community & organizational resources for clients and families. Onboarding of additional services and resources to increase success in the home. Health education to promote optimum prosperity.

Our Services Include:
Skilled Nursing
Acute and chronic disease management, including client and caregiver education. Assess and assist with disease processes & medications, including IV antibiotics, wound care, and tube feedings.
Care Management
Developing a home and/or health care plan. Navigating the healthcare system. Coordination of care services. Assistance with advanced directives, insurance issues and doctor visits. Assessing for resource availability and benefit, medication compliance and education, understanding the disease process and tools to manage chronic conditions.
ADL Assistance
Assistance with activities of daily living (ADLs), such as bathing, dressing, meal prep, light housekeeping and errands including prescription pick-up and delivery.
